I'm not sure I can help on this initial problem, too little
information for a post mortum - but this one...

There is one known problem where a mount with quota enabled
has caused a panic on a local QA system, also with a NULL
pointer dereference which may be causing this problem. I'm
having the devil's own job trying to reproduce it - but will
CC you when I do have a fix for it - may solve your problem.

If you survived the mount (with quota enabled), then this is
not the problem and there are no other known problems with
XFS quota, so we'd need to extract some more information to
diagnose further.
