[patch] 2.4.18-pre1 replace .text.lock with .subsection

This patch against 2.4.18-pre1 replaces section .text.lock with
.subsection. Using a special section for out of line code can generate
dangling references to discarded sections, the dangling references are
flagged as an error by binutils 2.11.92.0.12 onwards. See thread

Normally I would do this against 2.5 and backport to 2.4 but this bug
is affecting 2.4 users now.

The patch hits several architectures but only i386, arm and m68k are
really affected, the other changes are removing dead lines. i386 has
been tested, I need arm and m68k users to check that the generated code
is valid.

ia64 is still using .text.lock, the version of gas that I use
(2.96-ia64-000717) makes a complete mess of .subsection. I will look
at converting ia64 next year, after finding a version of gas that
handles subsection correctly.

The patch consists of several semi-independent patches, broken up and
commented for readability.

Standard macro for converting #define to a string. Don't want to use
MODULE_STRING, include module.h pulls in all the module definitions as well.

Add KBUILD_BASENAME to assorted Makefiles, it is used to generate
source specific labels to help in debugging lock problems.
