[BKPATCH] further BKL cleanups

Now that we have <asm-*/smplock.h> remove and everything in
<linux/smp_lock.h> we can move the definition/export of kernel_flag
to common code. Also cleanup smp_lock.h a little more.

You can import this changeset into BK by piping this whole message to:
'| bk receive [path to repository]' or apply the patch as usual.
