[PATCH] Remove useless include/asm-*/floppy.h code

Hi,
Attached patch removes some useless code.  Should apply to 2.4.19-pre10 and
2.5.21.  Not sure if the code is still in relevant arch trees, but various
people cc'd so that it can be eliminated.
